Masterclass: Exposure-based Incident Investigation

  • The importance of investigating incidents based on potential
  • Managing the biases that challenge real learning
  • What constitutes a REAL corrective action?
  • Responding to incidents with a deliberate focus on creating culture

This interactive masterclass run by Warren Smith from DEKRA will challenge your thinking about the why, when, and how of deep-dive incident investigations, and how best to link investigation outcomes with exposure reduction efforts. This session’s focus will not deal with the technical merits of various investigation methodologies such as ICAM, Taproot, or Why Tree. While that would be a fascinating discussion for another time, Warren will discuss how best practice organisations are evolving their incident response focus to be more proportionate to potential and focused on culture-creation, and about how organisations can move toward for effectively living the mantra of ‘never letting that incident happen again’.
